الاختبارات السريعة أداة قوية: نتيجة خلال دقائق داخل العيادة دون إرسال العينة لمختبر خارجي. لكن قوتها مشروطة بالاستخدام الصحيح، وأغلب النتائج المضللة سببها خطأ في الإجراء لا عيب في الاختبار.

التخزين: الخطأ الأكثر شيوعاً

أغلب الشرائط تُحفظ مبرّدة أو في درجة حرارة الغرفة حسب تعليمات الشركة، لكنها تحتاج جميعاً أن تصل إلى درجة حرارة الغرفة قبل الفتح. فتح كيس بارد في جو رطب يسبب تكثفاً على الشريط ويفسد النتيجة. أخرج الاختبار قبل الاستخدام بالمدة الموصى بها واتركه مغلقاً حتى يعتدل.

حجم العينة والقطرات

زيادة القطرات لا تعني نتيجة أوضح، بل قد تغرق الشريط وتمنع الهجرة الشعرية الصحيحة. ونقصانها يعني ألا يصل السائل إلى خط التحكم. استخدم الماصة المرفقة والتزم بالعدد المذكور حرفياً، وتأكد من ظهور خط التحكم دائماً — غيابه يعني اختباراً غير صالح مهما كانت بقية النتيجة.

توقيت القراءة

لكل اختبار نافذة قراءة محددة. القراءة المبكرة قد تُظهر سلبية كاذبة لأن التفاعل لم يكتمل، والقراءة المتأخرة قد تُظهر خطوطاً باهتة تُفسَّر خطأً كإيجابية. اضبط مؤقتاً والتزم بالنافذة المذكورة في النشرة.

متى تحصل السلبية الكاذبة؟

أشهر سبب هو الفحص في وقت مبكر جداً من العدوى، قبل أن يصل مستوى المستضد أو الأجسام المضادة إلى عتبة الكشف. كذلك قد تسبب العينة المخففة أو المخزّنة بشكل خاطئ نتيجة سلبية. لهذا لا تستبعد الاشتباه السريري بناءً على اختبار سلبي واحد، وأعد الفحص بعد الفترة الموصى بها إذا استمرت الأعراض.

ومتى تحصل الإيجابية الكاذبة؟

قد تنتج عن تداخل مع أجسام مضادة من التطعيم الحديث، أو أجسام مضادة أمومية في الصغار، أو تلوث العينة. الإيجابية في حيوان بلا أعراض تستدعي التأكيد بطريقة أخرى قبل بناء خطة علاجية عليها.

الاختبار السريع لا يلغي المختبر

اعتبر الاختبار السريع أداة فرز سريعة توجّه القرار الأولي، لا بديلاً كاملاً عن التحاليل المخبرية أو تقنيات مثل PCR عند الحاجة إلى تأكيد أو تحديد النوع. الجمع بين الأداتين هو ما يعطي أفضل النتائج.

Rapid tests are a powerful tool: a result in minutes, in-clinic, without shipping a sample to an external lab. But that power depends on correct technique, and most misleading results come from procedural error, not a faulty test.

Storage: the most common mistake

Most cassettes are stored refrigerated or at room temperature depending on the manufacturer, but all of them must reach room temperature before the pouch is opened. Opening a cold pouch in humid air causes condensation on the strip and ruins the result. Remove the test the recommended time in advance and leave it sealed until it equilibrates.

Sample volume and drops

More drops do not mean a clearer result. Flooding the strip prevents proper capillary migration. Too few drops mean the fluid never reaches the control line. Use the supplied pipette, follow the stated drop count exactly, and always confirm the control line appears. Its absence means an invalid test, regardless of what else you see.

Read timing

Every test has a defined read window. Reading too early can produce a false negative because the reaction has not completed. Reading too late can produce faint lines misread as positive. Set a timer and stay within the window given in the insert.

When do false negatives occur?

The most common cause is testing very early in infection, before antigen or antibody levels reach the detection threshold. Diluted or improperly stored samples can also read negative. Never dismiss clinical suspicion on the basis of a single negative test. Repeat after the recommended interval if signs persist.

And false positives?

These can arise from interference with antibodies following recent vaccination, from maternal antibodies in young animals, or from sample contamination. A positive result in an asymptomatic animal warrants confirmation by another method before a treatment plan is built on it.

Rapid tests do not replace the lab

Treat rapid tests as a screening tool that guides the initial decision, not a full substitute for laboratory panels or techniques such as PCR when confirmation or typing is required. Combining both is what produces the best outcomes.

A quick pre-test checklist

Check the expiry date, confirm the strip has reached room temperature, verify the desiccant sachet is intact, use the correct sample type (whole blood, serum, or plasma), set a timer, and confirm the control line at read time.
